Cause:
Firmware reset to factory defaults. Fix:
- Insert USB into DVR.
- Go to Menu → System (or Maintenance) → Upgrade/Firmware Upgrade.
- Select the firmware file shown on the USB and start the upgrade.
- Do NOT power off the DVR or remove the USB during the process.
